Parish Verification Form

The Archdiocese of Atlanta defines active parishioners as:
■ those who have been registered in the parish for at least six months,
■ attend Mass weekly and on Holy Days of Obligation,
■ contribute financially (in a trackable way) to support 1) the parish offertory and/or the Cathedral Fund* and 2) the Archbishop’s Annual Appeal,
■ and contribute time and talent by participating in at least one ministry in the parish

Active membership is determined by the parish in which the family is a member.

* These directly support the Cathedral and its ministries.

This form only applies to the current year. A new form has to be submitted at the beginning of every school year to determine eligibility for the Catholic rate for that year. To preserve your privacy during the admissions process, please fill out a separate form for each school you’re applying to.
